Applicants for RAFT memberships, please complete all sections of the application. You can either download the application and bring it into RAFT, or complete and send via email. In order for your application to be completed and a membership card to be issued, you will need to provide proof of your affiliation, with an education institution or a non-profit institution. Please mail proof, along with your payment to:

You will be asked to submit your membership payment on the next page. If you want to renew your membership use our renewal form.
Once we have received the membership application, proof of affiliation and payment, your membership will be mailed to your residence, within 10 business days.
